Cool Any ides on what is wrong?

This morning I noticed that my 22 year old geldings muzzle is wrinkled up, and his left nostril is pushed in. Yesterday he had the airplane ears going, but I thought the gnats were bothering him. Any help with what could be going on would be helpful. Have a call into my vet, but she hasn't called back yet. Thank you.
